Southwark and Lambeth Integrated Care Citizens' Forum

The forum is open to anyone who lives, works or studies in the two boroughs, or anyone who is interested in health and social care.

There will be opportunities to find out more about integrated care for older people and for adults with long term conditions.

There will also be a focus on asking how you want to influence the development of integrated care, and how you want to be involved in the future.
